• VA School Entrance Health Form​ -- please update our school records each time your child receives immunizations.
   

• MAT Medication Consent Form -- these completed forms are valid for up to 10 days (OR for emergency medications required on site: these completed forms are valid for up to 12 months when signed by a physician).

• Authorization Form for OTC Topical Medications (sunscreen, diaper cream, bug spray) -- these completed forms are valid for up to 12 months.

​​

• Developmental History -- feel free to update this anytime your child is transitioning to an older classroom or has had a prolonged (several months) absence.
   

• Infant Individualized Habits Sheet -- parents of infants can update this sheet at any time as your baby grows into new sleeping and eating routines.

• 2022-2023 Academic Calendar --download a hard copy of our September 2022 to August 2023 school year calendar. 

 

• Dietary Restriction Form--this form must be completed by both the parent and child's physician. This form is valid for up to 1 year, unless the parent/guardian provides us with a written request to discontinue the dietary restriction(s). ​​
